This Privacy Policy explains how Lida Lisowe, PLLC ("I," "me," or "my") handles information collected through lidalisowe.com (the "Site"). I care about your privacy, and I have tried to keep this straightforward.
Please do not share sensitive health information through this Site. The contact form and email are convenient, but they are not secure channels. Please keep clinical details, diagnoses, medication information, and other sensitive health details out of them. Once we are working together, we will use the secure client portal or another appropriate method for anything confidential.
Information collected
Information you provide. When you fill out the contact form, email me, call, or text, you may share your name, email address, phone number, the reason for your inquiry, and anything you choose to write in your message. You control what you send.
Information collected automatically. Like most websites, this Site may automatically collect limited technical information, such as your IP address, browser type, device type, the pages you visit, how long you stay, and the site that referred you. This information is general and is used to understand how the Site is performing.
How your information is used
Information collected through the Site is used to:

HIPAA and this website
This Privacy Policy covers the Site only. It is not my Notice of Privacy Practices.
As a licensed clinician, I am required to maintain the privacy of protected health information under HIPAA and applicable North Carolina law. Clients receive a separate Notice of Privacy Practices describing how protected health information is used and disclosed in the course of treatment, along with your rights regarding that information. If you are a client and would like another copy, please ask and I will provide one.
An inquiry submitted through this Site, on its own, does not create a treatment relationship or a clinical record.
Cookies and analytics
The Site may use cookies, which are small files stored by your browser, to help the Site function properly and to understand general usage patterns. If analytics tools are in use, they collect aggregated information about visits rather than identifying you personally.
Most browsers let you refuse or delete cookies through their settings. Some parts of the Site may not work as smoothly if you turn cookies off.

Inquiries are kept only as long as needed to respond and to meet any applicable legal, professional, or recordkeeping requirements, then disposed of appropriately. Clinical records, if a treatment relationship begins, are governed by my Notice of Privacy Practices and applicable law rather than by this policy.
Your choices
You may ask what information I have from your Site inquiry, ask that it be corrected, or ask that it be deleted, and I will honor reasonable requests unless I am required to retain it. You can also simply choose not to submit the form and call me instead.

This Site is intended for adults. My practice serves adults, and the Site is not directed to children. I do not knowingly collect personal information from children. If you believe a child has submitted information through the Site, please contact me and I will delete it.

These Terms are governed by the laws of the State of North Carolina, without regard to its conflict of law rules. Therapy services are provided only to individuals physically located in North Carolina at the time of service, consistent with my license. EMDR consultation services for clinicians are professional consultation rather than therapy, and are not licensed clinical care.

These Terms may be updated from time to time. The "last updated" date at the top of this page reflects the most recent version. Continuing to use the Site after a change means you accept the updated Terms.
